Stage 1 Wounds are from constant rubbing. Having fluids on the skin which rubs back & forth cause shear force & breaks down skin layers. The skin's appearance is red with skin layers cracking or early, tears which maybe several. Stopping at this stage is critically important.
From constant pressure, blood flow is decreased causing multiple layers of skin to die and be easily torn away. The top layers of subcutaneous
tissue are next to have reduced blood flow and start to die off but these are the important 'fat' covering muscles then deeper organs and bones which can later become involved.
Due to loss of blood flow to the area from constant pressure, every skin layer is torn away of the affected area plus, a large part of the subcutaneous tissues are also damaged in Stage 3 and holes appear or tunneling. If the wound continues without being addressed the muscle will be affected next and parts of the muscle will lose blood flow and start to die in Stage 4.
Prevention & treatment involves multiple Steps:

5. When area is thoroughly clean & has been wiped last time w/ clear water, allow area to dry then wipe w/ these lanolin based cloths.

For Stages 2-4 patients may have Duoderm or Dressing Changes. Follow the orders written by the practitioner.
Duoderm should only be changed when it is falling off or dirty. Put paper-tape along edges to prevent sheer force from pulling it lose.
To prevent skin breakdown around a stoma, keeping it clean & removing the salts, moisture & bacteria, which comes up through skin pores, is key. Changing the Appliance at least every other day is extremely important, especially when it is hot & people are sweating more. In those circumstances changing more frequently is necessary. Please try the type I identified in this video, they allow the skin to get more air the the typical type of heavy appliances.
